If I lose weight using GLP-1 medications, will I need to continue using them forever?

What is the ideal BMI before surgery, and why does it matter?

While candidacy for surgery is individualized, many patients experience safer procedures and smoother recoveries at lower BMI ranges. In our experience, outcomes are often optimized when BMI is approximately 27–28 or below, when appropriate and achievable.


Lower BMI is associated with:

  • Reduced surgical risk and anesthesia-related complications

  • Lower risk of infection and delayed recovery

What is the cost of GLP-1 medications? Can I use insurance?

Our program is designed to eliminate delays and provide a seamless, physician-guided experience. We offer access to only FDA-approved GLP-1 medications through transparent self-pay pricing models, typically ranging from $300–$500 per month depending on the medication and dose.
 

Some patients choose to pursue insurance coverage independently. This typically requires:

  • Working directly with your primary care physician or other prescribing provider within your insurance network.

  • Meeting specific BMI or medical criteria

  • Navigating prior authorization requirements
     

Some patients prefer this route, while others choose self-pay for speed, simplicity, and predictability.

How long is the weightloss/metabolic optimization program before surgery?

The timeline is individualized, but most patients participate in a 3-to-6-month program depending on their goals and starting point.

  • 10–20% of total body weight over 6–12 months
     

For example, a patient starting at 180 lbs may lose approximately 9–18 lbs in the first 3 months, with continued progress over time.

Nutrition & Muscle Preservation
 

During weight loss, our priority is not just losing weight but also preserving lean muscle mass and supporting recovery.
 

We emphasize:

  • High-protein intake to maintain muscle and support tissue repair 

  • Strength training / resistance exercise to preserve lean body mass

  • Prevent excessive metabolic slowdown by avoiding rapid, nutritionally depleted weight loss
     

Protein plays a dual role, supporting both wound healing and muscle preservation. It provides the building blocks for collagen and tissue repair while also helping maintain lean muscle mass during weight loss to preserve your basal metabolism. Both are essential for optimal surgical outcomes and recovery.
 

This foundation helps ensure that weight loss enhances and does not compromise your surgical outcome.

When do I stop taking GLP-1 medication before surgery? When can I restart it?

GLP-1 medications i.e. Zepbound, Ozempic, Monjaro, and Wegovy slow gastric emptying, which has important implications for anesthesia and surgical safety.

Before surgery:

  • We typically recommend holding GLP-1 medications for 1 week prior to surgery

  • This reduces the risk of aspiration during induction of anesthesia
     

After surgery:

  • Medications are usually resumed once you are tolerating a regular diet without significant nausea or vomiting

  • This is often 1–2 weeks after surgery, depending on recovery

  • If the medication is held for a short duration (≈1–2 weeks), patients can typically resume at their previous dose

  • If the medication has been held for >3–4 weeks, it is often better to restart at a lower dose and re-titrate to minimize side effects (particularly nausea)
     

Your  individualized plan will be based on:

  • Duration of interruption

  • Prior tolerance and dose

  • Postoperative recovery status
